Birth Affidavit
An Affidavit of Birth is a sworn statement, made under oath, attesting to the facts of a person’s birth. It is often used when an official birth certificate is unavailable, incomplete, or was registered late.
It entails: :
- Sworn Statement : The person making the affidavit (the affiant) declares that the information provided is true and accurate to the best of their knowledge. This is usually done before a notary public or another official authorized to administer oaths.
- Details of Birth : The affidavit typically includes crucial information about the birth, such as:
o The full name of the person whose birth is being attested to.
o Date of birth.
o Place of birth (city, state, country, and often the name of the hospital or location).
o Full names of both parents (if known).
o Affiant’s Knowledge : The affidavit should explain how the affiant has personal knowledge of the birth. This is important for its credibility. Often, the affiant is a parent, older sibling, or another close relative who was present at or shortly after the birth. It could also be the attending physician or midwife - Reason for Affidavit : The document often states why a birth certificate is not available or why the affidavit is being provided.

Eligibility Conditions
- Absence of Original Birth Certificate
- A child, under 18 years, being birthed in Morocco by Liberian parents (documents supporting this claim)
- Applying for identification documents, enrolling in educational institutions, and accessing healthcare services (supporting documents)
Required Documents
- Affidavit of Birth Application Form :
- Available at the embassy or downloadable online
- Proof of Identity : National ID card or any other official document.
- Recent Passport-Style Photo: Full face on a white background.
- Declaration of Loss or Theft : If applicable.
Proof of Citizenship: Applicant or parents of Applicant.
Application Procedure
- Prepare Documents: Ensure that all required documents are complete
- Administrative Fees Payment: 400.00 dh. The fee must be deposited in advance via bank transfer or direct deposit into our account
- Submit the Application: Present your file to the Embassy of Liberia in Rabat
- Application Processing: Processing time is usually 2 business days
- Affidavit of Birth Collection : Once approved, collect your document at the embassy
Important Information
- Fees paid are non-refundable.
- Any incomplete application will not be processed.
- Affidavit of Birth fee must be paid at the bank in the Embassy of Liberia
- Consular Fee Account: (011 810 00000321 000 2556422).
- All payment slips must be scanned and sent to the Consular Service email (consularservices@liberianembassymorocco.ma) and hard copies attached to the original application documents.
